System Change Request Instructions and Form

Use the SCEIS System Change Request Form to ask for enhancements, new processes or other changes to the SCEIS system.

Instructions:
  1. Obtain your manager’s approval for the System Change Request.


  2. Click here to open a new email in your email program .
    Note: If you are using an internet-based email program such as Hotmail, Gmail or MSN mail, open the website and create a new email message. Paste the address " sceis.ecc.chgmgt@ciosharepoint.sc.gov " into the TO field.


  3. Use the following subject line: System Change Request - [Agency Code], inserting your own agency's code.


  4. Highlight and copy the Change Request script from below into the body of the email.


  5. Complete the form by filling in all fields.


  6. Copy and insert the email address below if needed, and click the send button to email the information to: sceis.ecc.chgmgt@ciosharepoint.sc.gov


  7. Within a day, we will confirm receipt of your System Change Request. Within five business days, we will update you about the review status of your requested change.

To assure that you are using the current form, look in the upper right corner for the revision date: "Rev 2009-06-30."
